To get your refund by mail, here’s all you’ll need:
1. A completed Satisfaction Guarantee form: Download PDF
2. The original UPC from your ZYRTEC® product package
3. The original purchase receipt dated between 2/12/09 and 5/31/09 with the price circled
Put all three items in an envelope with proper postage and send to:

Please allow 8-10 weeks for delivery of refund check.
